Debit MasterCard / ATM Application
There are 2 easy steps to apply for either a Debit MasterCard or an ATM Card.
- Please review the EFT Disclosure before proceeding to the application. The EFT Disclosure and Agreement is the contract which covers your and our rights and responsibilities concerning the electronic funds transfer services (EFT) offered to you by Plus4 Credit Union. This is your copy of the agreement, please print and retain for your records.
- Proceed to the Electronic Services Application and check the box for the type of card you are requesting.
